Certified DOT Physical Exams for Commercial Drivers

Aviva Health offers Department of Transportation (DOT) physical exams for commercial drivers in Douglas County who need to obtain or renew their medical certification. Our providers perform exams that meet Federal Motor Carrier Safety Administration (FMCSA) requirements.

Whether you’re applying for your first Commercial Driver’s License (CDL), renewing your medical certificate or changing employers, we’re here to help you complete your exam efficiently and get back on the road.

What Is a DOT Physical?

A DOT physical is a medical exam required for many commercial drivers. The exam helps determine whether you’re physically able to safely operate a commercial motor vehicle.

During your appointment, your provider will:

  • Review your medical history
  • Check your height, weight and blood pressure
  • Test your vision and hearing
  • Perform a urinalysis to screen for certain medical conditions
  • Review any medications you take
  • Complete a physical exam
  • Discuss any health conditions that could affect your ability to drive safely

If you meet the medical standards, you’ll receive a DOT medical certificate based on your individual health status.

What Should I Bring?

Please bring:

  • A valid photo ID
  • A list of your current medications
  • Glasses or contact lenses, if you wear them for driving
  • Hearing aids, if you use them
  • Medical records or clearance letters for certain health conditions, if applicable
  • Any forms required by your employer

If you have conditions such as diabetes, heart disease, high blood pressure or sleep apnea, you may need additional documentation before your certification can be completed.

How Long Does a DOT Medical Certificate Last?

Most drivers qualify for a medical certificate that is valid for up to two years. Depending on your medical history or certain health conditions, your provider may issue a certificate for a shorter period and recommend follow-up evaluations.
