Patient and Family Advisory Council
Mission and Vision Statement
Aviva Health’s Patient and Family Advisory Council (PFAC) is a partnership of patients and family advisors and Aviva Health’s staff. The council is dedicated to enhancing comprehensive and compassionate patient and family-centered health care.

The PFAC’s vision is to prioritize efforts and institute tactics that will promote a whole-person approach to health and wellness for Aviva Health’s patient population. The council is committed to aligning improvement activities with the core attributes of the Patient-Centered Primary Care Home Program model – accessible, accountable, comprehensive, continuous, coordinated, and patient and family centered.
Goals and Objectives
- To promote and strengthen collaboration between patients and families and Aviva Health
- To provide a platform for receiving, processing, and responding to feedback from patients
- To establish an effective, systematic mechanism for improving the quality of health care deliverables and outcomes by incorporating feedback and decisions made by the council
Membership Structure
The Patient and Family Advisory Council will consist of nine to 11 members:
- 1-3 patient/family representatives from Roseburg, Sutherlin, Glide, and Myrtle Creek medical clinics
- Chief Medical Officer (co-chair)
- Director of Operations
- Clinical Quality Manager (co-chair/advisor and staff liaison)
- Compliance Risk Manager
Additionally, other staff members may be invited to meetings depending on agenda items. This may include board members, executive leaders, directors, managers, providers, nurses, and clinical and administrative support staff.
Join the Patient and Family Advisory Council
Aviva Health values the insights and experiences of its patients and their families. Joining PFAC is a great way to ensure strong communication between Aviva Health and the people we serve and help the organization meet the diverse and changing health care needs of the community. Individuals interested in joining PFAC at Aviva Health must meet the following qualifications:
- Candidate must be an established patient or family member/caregiver of an established patient who is receiving services at Aviva
- Candidate must be 18 years of age or 15 years of age with a parent/guardian present at meetings
- Preferred – patients who have utilized more than one service line or type (i.e., adult patient receiving medical and dental care who has a child receiving pediatric care within the organization)
- Candidates may be nominated or recommended by Aviva Health staff members
- Candidates must fill out a PFAC application describing his/her interest in becoming a patient
- After submission of completed application, candidates will be interviewed and selected by both council co-chairs
- New advisors will attend PFAC orientation and will satisfy all required trainings as a volunteer of Aviva Health prior to attending their first PFAC
- Advisor removal will be unanimously decided by both co-chairs
