Mobile Medical Services
Better Health Coming Your Way
Aviva Health operates two mobile medical units assigned to us by the Douglas County Commissioners to provide care to communities around Roseburg who do not have access to reliable health services.
Our mobile medical team is excited to bring access to health care to the people of Diamond Lake, Oregon, the second Friday of each month following, from 8 a.m. to 4 p.m., weather permitting.
Aviva Health also provides mobile medical services on the campus of Umpqua Community College the first Monday of each month. Known as the Hawk Health Center, the clinic is located outside HNSC and offers services from 10 a.m. to 3 p.m.
Call (541) 672-9596 to schedule an appointment. Our mobile team provides care to people of all ages – children, adults, and senior citizens.
Aviva Health received the mobile medical units in the midst of the COVID-19 pandemic. During that time, we used the “clinics on wheels” to offer COVID-19 vaccinations and testing across Douglas County.
The mobile team is eager to increase usage of these important assets to ensure people across Douglas County have access to important medical services to help them live healthier lives.
Aviva Health proudly serves Oregon Health Plan members and accepts most commercial insurances. For individuals who are uninsured or under-insured, services are provided on a sliding-fee scale based on monthly income. For a complete list of insurances accepted by Aviva Health, click here.
