Aviva Health, the largest Federally Qualified Health Center (FQHC) in Douglas County, operates its main clinic in Roseburg, designated as a Tier-5 Patient-Centered Primary Care Home. For residency and advanced practice provider training, the primary clinical site is the Myrtle Creek clinic, a Tier-4 Patient-Centered Primary Care Home that offers a more rural, community-based training experience.
Founded in 1992, Aviva Health is an independent, Board-governed, not-for-profit organization dedicated to providing high-quality, patient-centered, accessible, and affordable medical, dental, and behavioral health care to the people of Douglas County, OR, regardless of their ability to pay or insurance status. It delivers integrated services across eight clinic sites in Myrtle Creek, Roseburg, Sutherlin, North County, and Glide, including a Teen Health Center located on the campus of Roseburg High School and two mobile medical units. Aviva Health serves more than 13,500 Douglas County residents and averages nearly 50,000 patient visits annually.
The Myrtle Creek clinic supports a continuity of care model where patients of all ages receive comprehensive, team-based care. This clinic provides advanced practice providers and residents with a unique opportunity to train in a setting that meets the healthcare needs of a rural, underserved community.
