Aviva Health, the largest Federally Qualified Health Center (FQHC) in Douglas County, partnered with sole-community hospital CHI Mercy Health-Mercy Medical Center to establish the Roseburg Family Medicine Residency (RFMR) Program. Aviva Health, a Tier-5 Patient-centered Primary Care Home, serves as the family medicine practice for the residency.
Founded in 1992, Aviva Health is an independent, Board-governed, not-for-profit organization that cares for the people of Douglas County, OR, by providing high-quality, patient-centered, accessible and affordable medical, dental and behavioral health care, regardless of their ability to pay or insurance status. It offers integrated services from eight clinic sites in Roseburg, Myrtle Creek, Sutherlin, North County, and Glide, including a Teen Health Center located on the campus of Roseburg High School and two mobile medical units. Aviva Health provides care to more than 13,500 Douglas County residents and averages nearly 50,000 patient visits annually. RFMR residents care for patients of all ages in continuity at Aviva Health over the course of the three-year residency program.
