Apply to the Roseburg Family Medicine Residency Program
Applicants should submit their completed application through the Electronic Residency Application Service (ERAS). The Roseburg Family Medicine Residency ACGME Program Code is: 1204000732, Family Medicine OPTI West (Roseburg) Program. Applicants will be contacted via ERAS if they are invited for a personal interview.
Applicants should include the following items in their ERAS application:
- Personal Statement that indicates why you chose to pursue family medicine and why the Roseburg Family Medicine Residency Program is a good fit for you
- Three Letters of Recommendation with at least one from a Family Medicine Physician
- USMLE or COMLEX Scores
- Transcripts

Additional Information Regarding Applicant Criteria:
- All candidates must be eligible for employment at Aviva Health at the time of hire
- All candidates must qualify for a limited medical license through the Oregon Board of Medical Examiners
- Candidates graduating from a medical school outside of the United States or Canada prior to 2020 must have a complete, valid ECFMG certificate at the time of application
- Only ERAS applications will be accepted
- All applicants will be selected through the NRMP Match
We have a three-year graduation year cut-off date - We have no minimum score requirements for USMLE or COMLEX exams, passing on the first attempt is preferred
- Graduates from recognized international schools are welcome to apply so long as they have satisfied ECFMG requirements, apply via ERAS and register with the NRMP.
- At this time we are not equipped to offer visa sponsorship. All international medical graduates applying must be United States citizens or permanent residents to be considered.
